30 minutes: Does it take you 30 minutes to fall asleep at the beginning of the night or you are awake for 30 minutes in the middle of the night or you are waking up at least 30 minutes before you intend to and can't get back to sleep. 

3x per week or more - Is the above happening 3 times per week or more?

AND

3 months - If the above has been going on for three months, this is a signal to speak to your doctor and seek help.
